Getting started – how it works!

A friendly home care specialist visits to answer your questions about services, how funding works and how we can work with you to get the care & support you need.

3. Meet your care manager

Within a week, your care manager will visit to complete an in-depth care planning session to ensure you get the most out of your care funding an care achieve your goals.

4. Your care plan is finalised

Your care manager compiles a detailed profile all about your care needs, based on your health records, individual care planning sessions, your MAC assessment and more.

5. Services start without delay

Your care coordination team begins rostering all of the services  – no delays or waiting – so you can remain well and independent at home with the support you need.
