Your care manager compiles a detailed profile all about your care needs, based on your health records, individual care planning sessions, your MAC assessment and more.

5. Services start without delay

Your care coordination team begins rostering all of the services  – no delays or waiting – so you can remain well and independent at home with the support you need.
